  2. Signed Dispatch / Carrier Agreement 
  3. Limited Power of Attorney
  4. Copy of Carrier’s Authority 
  5. Copy of your W-9 
  6. Copy of CDL license (for each driver being dispatched)
  7. Copy of insurance certificate. (We require $100,000 in Cargo and $1,000,000 in Liability)
  8. Credit Card Consent Form
